Can someone please explain Semi-Perfect/Perfect Gems?

All topics I've seen regarding this subject leave me just as confused as when I started reading them. How are they made? Is it possible to mess up gems? Why can't I just keep combining untill I get one? I'm sorry if these things have already been answered in another topic, but if they have I didn't see it or I didn't understand it.

Semi perfect: Gem with a rating of 999,999
Perfect: Gem with a rating of 2,999,997 (3 semi perfects)
Imperfect: Gem with rating between 1,000,000 - 2,999,996 (Any gem between a semi perfect and a perfect)
Sotherogue: the greatest ever

To answer questions, they are made by combing gems to reach 999,999 with totem bonuses. Three of those are made into a semi perfect. You can't just keep combining, because once a gem is greater than 999,999, you can no longer put it in the totem, thus meaning a perfect gem is the max, after the vault was introduced.
